Who Are We?
Partners4Care is a family ran Stockton-on-Tees based complex care provider established in 2016 which focuses on providing client tailored care through its complex and supported living services. All of its operations are focussed around a core set of beliefs:

- Shared ownership of issues – Promoting personal inclusion in a supported way.
- Reliability – We know that people rely on us as a service which is why we always make sure to put people first.

How do I make a referral for one of your services?
To make a referral please contact our office team at 01642 988388. From here, we will collect all of the necessary infomration we need, assess the enquiry and return contact on whether or not we will be able to accomodate you request.
What areas does your complex care service cover?
Through our complex care service, we support clients primarily in Stockton-on-Tees and the surrounding areas including but not limited too Billingham, Thornaby and Yarm.
What exactly is complex care?
Our at home complex care service isn’t like a standard domicillary care service. It is designed for individuals living with learning disabilities, autism, neurological conditions and/or physical disabilities. To learn more about this service, please visit our service page.
