Personal Support Worker / Care Assistant Matching service


Partners 4 Care supports people to live independent, fulfilling lives by matching Personal Care and Support Workers (or specialist support workers) to people who need them to improve their well-being.


 We ensure the delivery of person-centred support, but we go further than this in that we promote independence and choices to enable customers to live the life they want, with services delivered in the way that they want. We like to call this ‘preference-focussed care and support’.

As well as being person-centred and preference-focussed, it’s relationship-led and delivered by your choice of carer.


Person-centred care needs to also be person-delivered care. It’s personal. You have your own support worker who helps to put you in control.


Wherever possible we avoid the limitations of a person’s condition. We actively seek to work in new ways with customers and develop new and interesting opportunities for each of them. 


So whether you want to listen to a bit of Elvis, get the bus to the betting shop (with a responsible budget), go to the cinema, or improve your social skills and ability to manage day to day tasks; it’s our job to facilitate that.


We do this by providing a support worker selection service and contract and HR management which benefits both the person receiving support and the worker.

How it works

The chosen worker is employed directly by the person receiving the support. Others call this person the ‘service user’, but we like to call them our valued customers.

Our role is to provide services to both the service user customer and the support worker. For every customer we provide an Individualised Care and Support Plan, so that both parties in the relationship are clear about what is being delivered.

Our customers can truly choose the type of care / support that they prefer. Customers include a variety of people with health, disability or social care and support needs. These needs usually arise from physical or mental health, physical or learning disability or age.


Supporting lifestyle aspirations 


Everyone has aspirations, regardless of their current circumstances. We make sure that these are included in what we do. For example, setting and achieving goals, however small. Making long term progress. Empowering people to live the life they want to live.


We stand up for the individual, who is sadly not often at the centre of care provision. We are changing that, one customer at a time.

Individualised Support Planning


We ensure individualised care and support, delivered at home, at residential care facilities and in the community. This includes personal care, social care and lifestyle and communications support. It also includes home help services such as housework (cooking, meal preparation, cleaning, laundry), day to day tasks such as administration, providing company and conversation and accompanying customers to appointments, social and leisure activities.
