Partners4Care is a family ran Stockton-on-Tees based complex care provider established in 2016 which focuses on providing client tailored care through its complex and supported living services. All of its operations are focussed around a core set of beliefs:

How do we ensure that we provide personalised support?
Providing quality personalised care starts with a support plan designed around the customer. Making sure that every aspect of care to be provided is carried out in a way that suits them and their needs. To do this, we don’t just take into account any medical diagnosis but personal independence, preferences, culture and background as well. Care plans are reviewed frequently in order to keep them up to date and relevant with the customers current needs.
